OcuSeal is a polymer hydrogel. Once the polymerisation process is initiated. i.e. when the powder and liquid components mix, the reaction cannot be slowed or stopped. With this in mind the OcuSeal must be applied to the desired area at
the end of the procedure in an uninterrupted fashion to avoid the hydrogel
polymerising in the applicator.

The polymerisation time of OcuSeal i.e. when the powder and liquid components are mixed, is 30 seconds for complete gel formation. The optimal texture for application to the ocular surface is a slightly more viscous liquid therefore the surgeon may prefer to wait 5 seconds once the mixing has taken place to get optimum ease of application.

If excess OcuSeal has been inadvertently applied to a greater area of the ocular surface than required, the excess can be simply wiped away while in a liquid form. Care must be taken when removing the excess once the gel has formed, as the entire film of Ocuseal covering the desired area may also be removed.

It is not entirely necessary to use the brush tip on the applicator. Many surgeons prefer to wait until the OcuSeal becomes slightly viscous and then merely drop the hydrogel over the desired surface.

It is ideal for the ocular surface to be as dry as possible before applying OcuSeal.

Pterygium surgery.

Without a graft: OcuSeal is not an adhesive, it is a sealant. For pterygium surgery without the use of a graft the surgeon must get good wound approximation of
the conjunctiva first with a few “stay” or “anchor” sutures and then apply a liberal amount of Ocuseal over the wound surface. If this is not done, the conjunctiva
will pull apart. Ocuseal reduces the number of sutures needed is pterygium
surgery by 40%.

With a graft: Ocuseal has been used with great success with auto grafts and amniotic grafts. The graft is secured in place with a few sutures and OcuSeal is then placed over the graft. Poor results are attained when the graft is placed over the Ocuseal.
